The liquidators of Health Squared Medical Scheme have called on members of the scheme to submit claims for the money due to them.
The High Court in Johannesburg placed Health Squared in liquidation on 17 February this year.
The Master of the High Court appointed Ralph Lutchman and Sophie “Peppy” Kekana of Concord Administrators as the liquidators.
Members who were owed money by Health Squared at the date of liquidation must submit claims on this form.
The completed claim form, together with supporting documentation, must be delivered by hand or courier to the liquidators’ office: Suite 201, Waterkloof Gardens, 270 Main Street, Brooklyn, Pretoria 0181.
